Platinum Health Navigation — Guidance. Advocacy. Peace of mind.

What we do

Practical, nurse-led support — from first questions to follow-through.

Each engagement is shaped around your family's circumstances. Most begin with one area of need and grow only as far as is genuinely useful.

  1. Nurse navigation

A registered nurse helps you make sense of the whole picture — what has been said, what has been planned, and what to ask next.

    This can include

    • Reviewing the care plan as your family understands it and identifying open questions
    • Translating medical terminology into clear, everyday language
    • Mapping the clinicians, appointments, and decisions involved
    • Suggesting practical next steps to raise with your care team
  2. Coordination of appointments, referrals & follow-ups

Appointments, referrals, and follow-ups organised carefully, so that nothing important slips between offices.

    This can include

    • Help scheduling and keeping track of appointments
    • Following through on referrals and outstanding results requests
    • Maintaining a simple shared timeline for the family
    • Preparing concise summaries and questions before visits
  3. Patient & caregiver education

Calm, plain-spoken explanations that help your parent — and the rest of the family — take part in decisions with confidence.

    This can include

    • Understanding information your care team has already shared
    • Preparing for upcoming visits, tests, or procedures
    • Organising medication lists to review with prescribing clinicians
    • Guidance for caregivers on what to observe and when to contact clinicians
  4. Advocacy & communication preparation

Support in preparing for important conversations, so your family's questions, goals, and preferences are clearly heard.

    This can include

    • Preparing focused question lists for appointments
    • Helping articulate goals, priorities, and preferences
    • Joining conversations where appropriate and agreed in advance
    • A clear debrief after key appointments
  5. Transitions of care

Thoughtful support around hospital discharge, rehabilitation stays, and the return home.

    This can include

    • Understanding discharge instructions and who to call with questions
    • Coordinating follow-up appointments after a hospital stay
    • Thinking through practical questions about readiness at home
    • Keeping family members aligned during a period of change

Scope of support

Clear about what we do — and equally clear about what we don't.

Within our scope

  • Helping families understand information from their care team
  • Organising appointments, referrals, follow-ups, and records requests
  • Preparing questions and supporting communication with clinicians
  • Practical guidance through hospital discharge and the return home

Outside our scope

  • Diagnosing conditions or recommending specific treatments
  • Prescribing or adjusting medications
  • Hands-on nursing care or in-home clinical services
  • Emergency or urgent response — call 911 in an emergency

Services are private-pay and are not billed to insurance. Scope and fees are discussed personally and agreed in writing before any paid work begins. Outcomes depend on many factors outside our control, and no particular result can be promised.
